This form is to be completed by any student who would like to schedule a meeting with a counselor (Ms. Moore or Mr. Ridd). 

Fill out the form completely. Please know that same day appointments may not be possible depending on the counselor's daily schedule, but we will get to you as soon as we can depending on the situation.
